This policy describes how information submitted through watersedgear.com is collected and used. It covers this website only. Information you share with your therapist in treatment is protected separately under HIPAA and our Notice of Privacy Practices, available at any of our offices.

When you submit our appointment request form, we collect the information you choose to provide: your name, phone number, email address, preferred office, the service you're interested in, and any message you include. We ask that you not include sensitive medical details in the form.

How we use it

We use your contact information for one purpose: to respond to your request and help you schedule care. We do not sell your information, share it with advertisers, or add you to marketing lists.

Who can see it

Form submissions are delivered to our administrative team. If we use a form-delivery or scheduling service to process submissions, that vendor handles your information only to deliver it to us. Our website may use basic analytics to understand site usage; analytics data does not include your form submissions.

Retention

We keep appointment request information only as long as needed to respond and schedule, after which it is handled under our standard records practices.
